How do you add the check boxes (good, fair, poor, etc.) to the template? I want to try it out and see how they look. I've seen them in some examples and I think I like them.

Go to "Report Settings" at the top menu bar and select "Ratings". Type in the ones you want and select "Check this box to include ratings in the report".

That should do it.
