It looks the way I thing it should look but it doesn't work quite yet.  tried to submit a comment while logged in.  It gives me the following comment: "Validation error, please try again. If this error persists, please contact the site administrator."
When I try to submit a comment when I am not logged in it tells me: "Access denied. You are not authorized to access this page." 
What do I need to change?

What page is the form copied onto? The original has to be published to work, though it doesn't have to be linked to a menu.

Ahh.  I didn't know it had to be published, neither did I know I could publish it but not have it on the menu.  Now it works fine when I am not logged in.  It still tells me to contact the site administrator if I try to submit a comment while I am logged in but that doesn't affect any potential visitors.  Thanks.

That's probably because it limits the number of submissions within a time period to prevent spamming. So when you're logged in you're just triggering it. Glad you got it working.

I am using theme 23 and upper left corner there was an ASHI logo, but all I see is and x.  How do I insert my logo or other picture

Under Themes, click on Configure next to 23 and the upload your image (I believe you're set now).
