This is a report template that I have been working on. ? Still a work in progress but it is getting close to the point that I'm happy with it.  PDF sample is also attached.

Thanks Scott, this looks great!! One suggestion I have is to change your header/footer format under Report Settings->Options so you still have page numbers. It looks like you went the white text route to hide the company name but the other header/footer format will still give you the text for page numbers.
