I received a request for a USDA inspection today.  It is a FIVE POINT Inspection, which is just like the 4-Point, but it also includes a structural inspection.  Anyone done this?  Have a template?  Or any information?  Thank you.

Haven't seen it before myself. You could create a structure add on to the 4 point. Did they tell you where you could find a form for it?

No, in fact, even USDA doesn't have a form!  So, I am kind of at a loss.

It looks like something that insurance companies ask for quite a bit. From what I read, most have a particular form they want you to use.

Yes, most of the time they do.  Thanks for the help.  I think I will just do an add-on to the 4 Point.

Matt,
I perform a few USDA inspections a year. I perform a standard home inspection, with a standard inspection report, which basically covers everything the USDA needs. If the home is on a well and/or septic, those need to be certified also. Typically, the regional office will have these services performed before they even call you for the home inspection (at least in my area). If you are looking to do these types of inspection, do yourself a favor and go down to the local USDA office and speak with them. They are very helpful with answering questions. Bring a sample report with you for them to review and "sign-off" on. Also, make sure to bring some biz cards with you!
