Hi Christopher,
Welcome to the boards!
Most of the guys just include it within their normal report by commenting in the area where the damage is found. Some states have special forms which we will be integrating in a future version. We have also finished and are finalizing the integration of a new drawing module to sketch out detailed floor plans and where the issues are.

Thanks Dominic. There is a standard form I was interested in. I will get you the actual form # when I can, but it is ok. I will have to do that in paper and fax it.

Florida uses form 1145 (a 2 pager that was instituted in May 09.) I'll attach a copy when I get it from my termite guy.

Yea I think that is it I will did 1 out tomarrow and maybe just scan it. My location of business and home are different and Nachi screwed that up I had to change everything to my business address. I will just say NY I almost have this report thing down, I am going to do a full sample report to show at presintations tomarrow, I am sure i am going to have 40 questions!  |

Definitely ask away. And don't forget this site is completely separate from NACHI, you can list anything you want here. We have members from all associations.
What guys do right now is full out the NMPA-33 in Adobe Acrobat and merge it with the Home Inspector Pro report and email the single file. Check and see if that's the form and let me know.
